User Agreement Waiver
TANNING BED USE AGREEMENT AND RELEASE OF LIABILITY
This agreement is entered into on January 15, 2024 between Tanning Kiosk Facility and the undersigned user.
1. ACKNOWLEDGMENT OF RISKS
I acknowledge that I have read and understand the potential health risks associated with tanning bed use, including but not limited to: skin damage, premature aging, eye damage, and increased risk of skin cancer. I understand that UV radiation can be harmful and that I should consult with a healthcare professional before using tanning equipment.
2. USE OF PROTECTIVE EQUIPMENT
I agree to use protective eyewear provided by the facility at all times during tanning sessions. I understand that failure to use protective eyewear can result in serious eye damage.
3. HEALTH CONDITIONS
I certify that I have completed the health survey truthfully and have disclosed all relevant health conditions. I understand that certain medical conditions and medications may make tanning unsafe for me.
4. RELEASE OF LIABILITY
I hereby release, waive, and discharge Tanning Kiosk Facility, its owners, employees, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by me or any property belonging to me while using the tanning facilities.
5. AGREEMENT TO TERMS
I have read this agreement fully, understand its terms, and agree to be bound by them. I acknowledge that I have had the opportunity to ask questions about this agreement.
